Crew Group, Inc. and its affiliates (collectively the "Company'') of consumer reports and investigative consumer reports (i.e. background check reports) prepared by a consumer reporting agency ("'Agency"). I understand that if the Company hires me, the Company may rely on this Authorization form to obtain additional reports on me from an Agency during my employment without asking for my consent again, to the extent permitted by applicable law. I also authorize disclosure to the Agency and its agents information needed to prepare a background report for the Company, subject to any applicable laws protecting my privacy, including but not limited to: my past or present employers; learning institutions, including colleges and universities; law enforcement and all other federal, state and local agencies; federal, state and local courts; the military; credit bureaus; drug and alcohol testing facilities; motor vehicle records agencies; all other private and public sector repositories of information; and any other person, organization, or agency with any information about or concerning me. I understand that an investigative report may be generated on me that may include information as to my character, general reputation, personal characteristics, or mode of living; work habits, performance or experience, along with reasons for termination of past employment, professional license or credentials; financial and credit history; or criminal civil driving record history. The information that can be disclosed to the Agency and its agents may also include, but is not limited to, information concerning my employment and earnings history, education, credit history, motor vehicle history and military service, I understand that General Information Services, Inc.(GIS) on behalf of the Company may be requesting information from public and private sources about any of the information described in this paragraph in connection with J.Crew's consideration of me for employment, promotion or position re­assignment or contract now, or at any time during my tenure with J.Crew, and give my full consent for this information to be obtained. Notwithstanding the foregoing, I understand that GIS will not investigate my credit history unless I am applying for a new hire or promotion into a position involving a payroll or finance department role with access to sensitive customer and/or banking financial information.II. If I am a California resident, I understand that the Agency will send me a copy of any consumer report or investigative consumer report at the same time that the report is made available to the Company. Under section 1786.22 of the California Civil Code, I understand that I may view the file maintained on me by GIS during normal business hours on reasonable notice. I may also obtain a copy of this file upon submitting proper identification and paying the costs of duplication services, by appearing at GIS in person or by mail. I may also receive a summary of the file by telephone. The agency is required- to have personnel available to explain my file to me and the agency must explain to me any coded information appearing in your file. If I appear in person, a person of my choice may accompany me, provided that this person furnishes proper identification.VI.
